Holy Name is excited to announce that beginning with the 2020-2021 school year, we will be utilizing FinalForms, an online forms and data management service. FinalForms will replace the paper forms you typically fill out at the start of the school year.
You will soon receive, if you have not already, an account confirmation email from FinalForms (Those families with fall student-athletes likely received this confirmation email previously. We ask that you please login in again to make sure all additional forms and added fields are filled in correctly). Please click the link provided in the account confirmation email and follow the steps to login and access your student(s). You can also utilize the Parent Playbook, which can be found by visiting www.holynamehs.com/finalforms, to help you get started. If you require any support during the process, scroll to the bottom of the FinalForms page and click “Use Support.”
All families will be required to use FinalForms, as it allows you to complete and sign enrollment, back-to-school and athletic participation forms for your students. The best news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! We ask all families to please fill out everything in FinalForms by Thursday, August 27.
This online system also pre-populates information wherever possible, for each of your students, saving you time. You may review your data at any time to verify it is current. You, as well as your student when applicable, will be required to sign forms once per year and after any update.
The only start-of-school-year documents that will not be available in FinalForms are the On-Campus Medication Permission Forms (ex. Inhaler Use, EpiPens, etc.) and the Parking Pass Application Form. To obtain those, please come to the Holy Name Main Office between the hours of 8 a.m. and 3 p.m. Monday through Friday. Or, you may call 440-886-0300 ext. 120 for more information.
Again, please fill out everything in FinalForms by Thursday, August 27. Additionally, and most importantly, no student will be permitted to enter the building on his/her orientation day if any information needed in FinalForms is missing. This is for the safety and well-being of all students and faculty members.
