FinalForms is Holy Name's online forms and data management service. To begin registering your student at Holy Name, you must create an account as soon as possible.
Please register at https://holynameparmaheights-oh.finalforms.com/ and follow the prompts to create your account, create your student's account, and sign your forms.
We are asking that the first 3 form sections be completed by the Welcome Meeting on Thursday, February 23. These sections are: Contact Information, Demographic Information, and Additional New Student Information. Feel free to complete as much as possible, but in order to register for classes at your student's guidance counselor appointment, the 3 sections listed above must be completed.

*Note: If you have a current Holy Name student and are already using FinalForms, please do not create a new account. You will only need to 'add' your new student to an existing account.
**Note: Your student will receive a student account in August at Freshmen Orientation. Please do not worry about entering a student email at this time.
To hold your seat in the Class of 2027, a $200 non-refundable registration fee is due by Friday, March 31st.
If you would like to pay online with credit card, you will do so through your Final Forms account. Please allow a few days after creating your account for the registration fee payment portal to be made available.
If you prefer to pay with cash, check (made payable to Holy Name High School), or money order, you may mail the fee or drop off your payment during school hours or bring it with you to the Welcome Meeting or your student's guidance counselor appointment.

The next step in the enrollment process is to meet with your student's guidance counselor, who will lead you through all aspects of Freshman Registration. During the individual appointment, you will register for your freshman courses and discuss your transition into high school. Remember, you will have the same counselor for the next four years. They are very excited to meet you and your family!
More information regarding the appointment will be covered at the Welcome Meeting on Thursday, February 23rd.
Our counselors will be available on the following dates for 20 minute time slots: Wednesday, March 1st, Thursday March 2nd, Monday March 6th, and Wednesday March 8th.

Hear from Holy Name's administration on what to expect as your student enters high school. This meeting is intended to help answer any questions you have and set you up for sucess.

Please fill this out even if you cannot attend so we have your student's t-shirt size!
If you are unable to attend, we will send you a packet via mail after the meeting.
The Team Shop will be open prior to the Welcome Meeting. Hours on Thursday evening will be 4:30 PM to 5:45 PM. Additionally, the Team Shop will be open on:

Tuesday, February 28: 11:30 AM to 2:30 PM

If your student took a high school accredited world language class everyday during their 8th grade year, Holy Name will transcript the class and place your student appropriately. To confirm your child's placement, they may be asked to take a foreign language placement exam.
If your student took world language on a weekly basis in grade school and would like to place into a 2nd year world language class at Holy Name, they must take a world language placement exam.
All world language placement exams will be administered after your family's registration appointment with their counselor.
If your student has yet to take the High School Placement Test (HSPT), they must register to take a make-up exam on Wednesday, March 29th from 4:30 - 7:30pm. All accepted students are required to have a test on file.

Holy Name High School accepts the Cleveland Scholarship Program (CSP). Provided through the Ohio Department of Education, this Program gives students living within the boundaries of the Cleveland Metropolitan School District the opportunity to attend private schools at a discounted rate. The high schools chosen for attendance must be located in Cleveland, or located in a district adjacent to Cleveland. Holy Name qualifies as it is located in the Parma City School District, which is adjacent to Cleveland. Please note that these scholarships are controlled by the state. Holy Name is here to submit your application but is not involved in the selection or distribution of the funds.
For high school students, the maximum scholarship amount awarded through the ODE's Cleveland Scholarship Program is $7,500. Families are then responsible for paying the remainder of the tuition balance.
Beginning in 2023, Holy Name will be utilizing Final Forms to streamline the state scholarship process. The parent/guardian will log into his/her Final Forms account to access the state scholarship information. The documents must be submitted on Final Forms as well. Your patience is appreciated as we transition to this new process.
*Uploading the materials to Final Forms in a timely manner is important so that Holy Name can submit them to the state. Per the state’s requirement, if a scholarship application is not updated to “Awarded” status PRIOR to the first day of the 2023-2024 school year, the amount that is awarded will be affected. *

The 2023-2024 list can be found here: 2023-2024 EdChoice Traditional Designated Public Schools List. This includes, but is not limited to, Bedford, East Cleveland Shaw, Euclid, Garfield Heights, Maple Heights, Richmond Heights, Warrensville Heights, Elyria, and Lorain High Schools.

The following information is needed in order to apply for a new or renewal scholarship:

Additionally, if you believe you would qualify for EdChoice Traditional – Low Income (Traditional EdChoice Scholarship applicants qualify for low income status if income is at or below 200% of the Federal Poverty Guidelines) you must fill out an Income Verification Form. We strongly encourage you to fill the Income Verification Form out using the online portal that can be found at https://ohid.ohio.gov/wps/portal/gov/ohid/login. To learn how to access the Scholarship Income Verification System, view this quick guide for help: ODE: How to Access Online Income Verification System.
If you are not able to upload your information online, you are still able to mail the following low income verification form to the Ohio Department of Education: 2023-2024 Income Verification Form. However, we do not recommend this option and encourage you to use the online portal if at all possible.
Families who qualify for EdChoice Traditional must pay the remaining tuition balance after the $7,500 scholarship is applied. However, families who have their income verified and qualify as EdChoice Traditional – Low Income do not have to pay the remaining tuition balance. Other fees will need to be paid.
All income verification documents and forms must go directly to the Ohio Department of Education; Holy Name is not involved in that review process.

EdChoice Expansion is another possibility for Holy Name students who do not live in the city of Cleveland, and who are not assigned to a public high school designated on the EdChoice Traditional List. EdChoice Expansion is based on gross family income.
Families are eligible to apply for the EdChoice – Expansion Scholarship if the family household income is at or below 250 percent of the Federal Poverty Guidelines. Here are the 2023-2024 EdChoice Expansion Income Guidelines.
Those families whose household gross income is between 201-250 percent of the Federal Poverty Guidelines are eligible to receive a $7,500 EdChoice Scholarship, but must pay the remaining tuition balance. EdChoice Expansion recipients that have a household gross income of 200 percent or lower qualify for low-income, and Holy Name cannot charge the remaining tuition balance (other fees are required to be paid to Holy Name).

Below are the scholarships now available to the Class of 2027! Online scholarship applications open on Thursday, February 23, 2023, and close Friday, March 24, 2023 at 11:59 PM. If a student is a recipient of this scholarship, he/she will be notified no later than May 2023.

Ciolek Educational Fund - The purpose of the Ciolek Educational Fund is to provide family members of the Donors (and certain other grantees) with funds to obtain an education at Holy Name High School, St. Mary’s Prep School in Orchard Lake, Michigan, or any approved Catholic high school in the State of Ohio. Applicants must be current eighth graders who will be incoming freshmen at Holy Name during the following school year. One (1) $1,500 renewable scholarship is awarded per class. All scholarships will be used for the following academic year.
Class of ’57 Scholarship - Patrons of the class of 1957, who came from many Catholic elementary schools, including Holy Name Elementary, established the Class of 1957 Scholarship Fund on September 16, 2007 during their 50th reunion, under the coordination of Joseph F. ’57 and Elaine M. Swyt. It is their intention through this fund to proclaim their faith to Holy Name by offering financial support to deserving students, so that they can carry on its rich tradition and spirit. Applicants must be current eighth graders at Catholic grade schools who will be incoming freshmen at Holy Name during the following school year. One (1) $1,000 non-renewable scholarship is awarded per class.
Patrick '71 and Mary Lee Gorbett Scholarship - Established in 2023 by Patrick ’71 and Mary Lee Gorbett, this scholarship was created to provide financial support for those families seeking a Catholic education at Holy Name High School. Patrick is a graduate of Holy Name, as are many of his relatives, and came from a large family of nine. Patrick and Mary Lee highly value the gift of a Holy Name education and understand how expensive a Catholic education can be with a larger family. This scholarship is intended for students who demonstrate a commitment to the ideals of Holy Name and the triple threat of mind, body, and spirit. The Patrick ’71 and Mary Lee Gorbett Scholarship will give first consideration to families with three or more children in elementary and/or high school. One (1) $5,000 renewable four-year scholarship will be awarded annually to an incoming freshman.
The Greater Cleveland Scholarship for the Urban Student at Holy Name High School - The Greater Cleveland Scholarship for the urban student at Holy Name High School was established in 2022 by an anonymous Holy Name alumnus. It is his desire to broaden the availability for financially underprivileged and academically qualified students with a college preparatory, morally sound education who would otherwise have a limited choice in furthering their education. This endowed scholarship is also in appreciation and recognition of the Sisters of Charity of Cincinnati. The alumnus credits much of his success and life achievements to the care and guidance he received as a student. Applicants must be current eighth graders who will be incoming freshmen at Holy Name in 2023-2024. One (1) $1,500 renewable scholarship will be awarded for the 2023-2024 school year.
Bernard & Anna Kilcoyne Scholarship - This scholarship was established in 1989 in honor of Bernard and Anna Kilcoyne. Grants will be awarded in the following priority: First, to the direct descendants of Bernard and Anna Kilcoyne, and second, to four non-family members, one each in grades nine, ten, eleven and twelve at Holy Name High School. The number of $500 scholarships awarded varies from year to year. All scholarships are renewable, and will be used for the following academic year.
Richard Koenig Memorial Scholarship - Established in 2016, the Richard Koenig Memorial Scholarship was created to honor the memory of Rick Koenig, who passed away in 2015. Rick was a loving husband to his wife, Deidre (Dee Dee) Maloney ’81 Koenig, and a devoted father to his three children, Ricky ’05, Ryan ’10 and Rachael ’14. He loved watching and cheering on his children and their teammates as they competed in wrestling, football and volleyball for the Green Wave. Rick epitomized what it means to be a ‘family man,’ and will forever be remembered for the love and respect he had for all those who were lucky to call him a friend. Applicants must be current eighth graders who will be incoming freshmen at Holy Name during the following school year. Two (1) $1,000 non-renewable scholarships are awarded per class.
LaMantia Scholarship - Established in 2022 by Joseph LaMantia, this scholarship fund was set up to ensure qualifying student(s) are provided financial assistance to support their Catholic education. The LaMantia Family is very committed to providing scholarships and opportunities to students pursuing higher education in STEM (Science, Technology, Engineering, and Math) fields. The LaMantia Family believes strongly in giving back to the community. They have been very fortunate to become successful, reaching the top tiers of Global 50 companies and starting technology businesses with global reach and impact. The LaMantia Family certainly hopes that scholarship recipients consider giving back to the community when they can with time, talent and treasure.
The application must be accompanied by a brief essay of no less than 200-250 words on why a Catholic education is important to him/her.
One (1) $500 renewable scholarship will be awarded to an incoming freshman student. The award will be applied beginning with the 2023-2034 academic year.
CHALTA Sprungle Memorial Scholarship - This scholarship is offered by the Cleveland High School and Academy Lay Teachers Association (CHALTA). Applicants must be current eighth graders who will be incoming freshmen at Holy Name during the following school year. The deadline for submission is Friday, April 21, 2023, and applications should be mailed back to Holy Name High School, Attn. Mrs. Kathryn Lechler, 6000 Queens Hwy. Parma Heights, OH 44130. Two (2) $500 scholarships will be awarded. Recipients will be notified by letter by mid-June 2023.

This scholarship is offered by the Cleveland High School and Academy Lay Teachers Association (CHALTA). Applicants must be current eighth graders who will be incoming freshmen at Holy Name during the following school year. The deadline for submission is Friday, April 21, 2023, and applications should be mailed back to Holy Name High School, Attn. Mrs. Kathryn Lechler, 6000 Queens Hwy. Parma Heights, OH 44130. Two (2) $500 scholarships will be awarded. Recipients will be notified by letter by mid-June 2023.

St. Elizabeth Ann Seton Council 11464 Knights of Columbus Ferdinand P. Goll Memorial Scholarship
This scholarship is offered to all students who are enrolled in a Catholic high school located within the Cleveland Diocese. One (1) $500 scholarship will be awarded. The application deadline is April 15, 2023. Click here to download the Goll Memorial Scholarship Application.
The Martin T. Galvin Memorial Scholarship Fund for Catholic High School Students
The Lawyers Guild of the Catholic Diocese of Cleveland established the Martin T. Galvin Memorial Scholarship Fund in conjunction with the Catholic Community Foundation. The Reminger Foundation granted $10,000 toward the creation of this scholarship to honor their departed colleague and friend Marty Galvin. The purpose is to provide scholarship assistance to a student enrolled or entering a high school providing a Catholic education within the Diocese of Cleveland. Students eligible to apply must have a family member who is a member of the Catholic Lawyers Guild or a parent who is an attorney practicing law in Northeast Ohio; be currently accepted to, or enrolled in a Catholic high school within the Diocese of Cleveland; be registered at a parish in the Diocese; and be participating members of their school and/or community. Applicants must complete the student-applicant sections of the application which includes a listing of extracurricular activities and a written essay. Students must also select someone to complete the letter of recommendation and ask their
principal to complete another section. If you meet these criteria and would like to apply, applications are available in the main office or with Mrs. Carter. All completed applications must be returned to Mrs. Carter by March 1.
All students are invited to apply for the newly established Angel Scholarship, a fund that provides tuition assistance to both current and incoming students. The deadline to apply is Monday, April 3, 2023.

Please note the following:
- In addition to turning the above-mentioned form in, you must have a completed FACTS financial aid application on file at Holy Name by April 30, 2023 for scholarship consideration.
- Completion of this application does not guarantee a scholarship award as funding is limited.
- Scholarships are awarded based on family financial need.
- One application can be submitted per family. Please make sure to list all students on the single application. A separate form for each individual student is not necessary.
- Receiving the Angel Scholarship does not affect a student’s ability to receive other scholarships or tuition assistance.
The Angel Scholarship Fund is made possible due to the recently established Tax Credit Scholarship Program in Ohio. Through this program, individuals may receive a 100% tax credit against their Ohio income tax liability, with a maximum credit of $750 ($1,500 for married couples filing jointly).
Tax Credit Scholarship Program dollars designated for Holy Name High School go into the Angel Scholarship Fund. This fund is being managed locally by the Catholic Community Foundation.
