FinalForms is Holy Name's online forms and data management service. To begin registering your student at Holy Name, you must create an account as soon as possible.
Please register at https://holynameparmaheights-oh.finalforms.com/ and follow the prompts to create your account, create your student's account, and sign your forms.
We are asking that the first 3 form sections be completed by the Welcome Meeting on Thursday, February 27. These sections are: Contact Information, Demographic Information, and Additional New Student Information. Feel free to complete as much as possible, but in order to register for classes at your student's guidance counselor appointment, the 3 sections listed above must be completed.

To hold your seat in the Class of 2029, a $200 non-refundable registration fee is due by Tuesday, April 1.
If you would like to pay online with credit card, you will do so through your Final Forms account. Please allow a few days after creating your account for the registration fee payment portal to be made available.
If you prefer to pay with cash, check (made payable to Holy Name High School), or money order, you may mail the fee or drop off your payment during school hours or bring it with you to the Welcome Meeting or your student's guidance counselor appointment.

The next step in the enrollment process is to meet with your student's guidance counselor, who will lead you through all aspects of Freshman Registration. During the individual appointment, you will register for your freshman courses and discuss your transition into high school. Remember, you will have the same counselor for the next four years. They are very excited to meet you and your family!
More information regarding the appointment will be covered at the Welcome Meeting on Thursday, February 27.

Hear from Holy Name's administration on what to expect as your student enters high school. This meeting is intended to help answer any questions you have and set you up for success.
Please RSVP here by Friday, February 14 and to let us know if you can attend and your student's t-shirt size.
Please fill this out even if you cannot attend so we have your student's t-shirt size!
If you are unable to attend, we will send you a packet via mail after the meeting.
The Team Shop will be open prior to the Welcome Meeting.
If your student took a high school accredited world language class everyday during their 8th grade year, Holy Name will transcript the class and place your student appropriately. To confirm your child's placement, they may be asked to take a foreign language placement exam.
If your student took world language on a weekly basis in grade school and would like to place into a 2nd year world language class at Holy Name, they must take a world language placement exam.
All world language placement exams will be administered after your family's registration appointment with their counselor.
Patrick and Mary Lee Gorbett Scholarship
Established in 2023 by Patrick ’71 and Mary Lee Gorbett, this scholarship was created to provide financial support for those families seeking a Catholic education at Holy Name High School. Patrick is a graduate of Holy Name, as are many of his relatives, and came from a large family of nine. Patrick and Mary Lee highly value the gift of a Holy Name education and understand how expensive a Catholic education can be with a larger family. This scholarship is intended for students who demonstrate a commitment to the ideals of Holy Name and the triple threat of mind, body, and spirit. The Patrick ’71 and Mary Lee Gorbett Scholarship will give first consideration to families with three or more children in elementary and/or high school.
The application is to be accompanied by an essay of 500 words or less, answering the following question:
Explain the extracurriculars that you are involved in currently. Why do you enjoy these activities? Then, share with the committee how your family has shaped and influenced your participation. Give us an example of how your family supports your particular interests, whether by attending your sporting events, ceremonies, or other extracurricular activities or traditions.

No late applications will be accepted.
The Angel Scholarship application process is open to all students at Holy Name. Scholarships are awarded based on family financial need. Completion of the Angel Scholarship application does not guarantee a scholarship award as funding is limited and demand may outweigh available funds.
One Angel Scholarship Fund application can be submitted per Holy Name family. Please make sure to list all Holy Name students on the single application.
Angel Scholarship applications are due back to the Holy Name Main Office by Friday, April 5, 2024. Paper copies of the form are located in the Main Office.
Additionally, your family must have a completed FACTS financial aid application on file for Angel Scholarship consideration by Tuesday, April 30, 2024.

CHALTA Sprungle Memorial Scholarship Application
The Cleveland High School And Academy Lay Teachers Association (CHALTA)
believes in Catholic education and the values which that system inspires. CHALTA desires that
as many people as possible within the Diocese of Cleveland participate in that education and its
benefits. Therefore, CHALTA is offering two $500 scholarships to incoming ninth graders at
each of the Diocesan owned Catholic high schools. CHALTA will base the selection of the students on their Academic Achievement, completion of the school selection statement, and a reference letter.
The deadline for submission of the listed items below is Friday, April 19, 2024. The
winners will be notified by letter by Friday, June 7, 2024.

Class of ’57 Scholarship Application
Patrons of the class of 1957, who came from many Catholic elementary schools, including Holy Name Elementary, established the Class of 1957 Scholarship Fund on September 16, 2007 during their 50th reunion, under the coordination of Joseph F. ’57 and Elaine M. Swyt. It is their intention through this fund to proclaim their faith to Holy Name by offering financial support to deserving students, so that they can carry on its rich tradition and spirit. Applicants must be current eighth graders at Catholic grade schools who will be incoming freshmen at Holy Name during the following school year. One (1) $1,000 non-renewable scholarship is awarded per class.
The Greater Cleveland Scholarship Application
The Greater Cleveland Scholarship for the urban student at Holy Name High School was established in 2022 by an anonymous Holy Name alumnus. It is his desire to broaden the availability for financially underprivileged and academically qualified students with a college preparatory, morally sound education who would otherwise have a limited choice in furthering their education. This endowed scholarship is also in appreciation and recognition of the Sisters of Charity of Cincinnati. The alumnus credits much of his success and life achievements to the care and guidance he received as a student. Applicants must be current eighth graders who will be incoming freshmen at Holy Name in 2023-2024. One (1) $1,500 renewable scholarship will be awarded for the 2023-2024 school year.
Richard Koenig Memorial Scholarship Application
Established in 2016, the Richard Koenig Memorial Scholarship was created to honor the memory of Rick Koenig, who passed away in 2015. Rick was a loving husband to his wife, Deidre (Dee Dee) Maloney ’81 Koenig, and a devoted father to his three children, Ricky ’05, Ryan ’10 and Rachael ’14. He loved watching and cheering on his children and their teammates as they competed in wrestling, football and volleyball for the Green Wave. Rick epitomized what it means to be a ‘family man,’ and will forever be remembered for the love and respect he had for all those who were lucky to call him a friend. Applicants must be current eighth graders who will be incoming freshmen at Holy Name during the following school year. Two (1) $1,000 non-renewable scholarships are awarded per class.
LaMantia Scholarship Application
Established in 2022 by Joseph LaMantia, this scholarship fund was set up to ensure qualifying student(s) are provided financial assistance to support their Catholic education. The LaMantia Family is very committed to providing scholarships and opportunities to students pursuing higher education in STEM (Science, Technology, Engineering, and Math) fields. The LaMantia Family believes strongly in giving back to the community. They have been very fortunate to become successful, reaching the top tiers of Global 50 companies and starting technology businesses with global reach and impact. The LaMantia Family certainly hopes that scholarship recipients consider giving back to the community when they can with time, talent and treasure.
The application must be accompanied by a brief essay of no less than 200-250 words on why a Catholic education is important to him/her.
One (1) $500 renewable scholarship will be awarded to an incoming freshman student. The award will be applied beginning with the 2023-2034 academic year.
The Fund a Dream Scholarship is offered to all K-12 students in the Diocese of Cleveland who demonstrate financial need. This scholarship is worth $2,500.

The Martin T. Galvin Memorial Scholarship is given annually to a Catholic high school student who has a family member who is a member of the Catholic Lawyers Guild or a parent who is an attorney practicing law in the Diocese of Cleveland.
To apply, please fill out the Galvin Scholarship Application.
The entire application - including the required essay and letter of recommendation - must be returned to Principal Mrs. Molly Krist at Holy Name by Friday, March 1, 2024. Mrs. Krist will then fill out the principal portion of the application.
EdChoice Expansion is another possibility for Holy Name students who do not live in the city of Cleveland, and who are not assigned to a public high school designated on the EdChoice Traditional List.
EdChoice Expansion Scholarships are awarded based on a family's Adjusted Gross Income (AGI). The state will determine a NEW applicant’s award by using the 2023 1040 tax form (pg. 1, line 11). For renewal applicants, the previous percentage calculated by the state will be used to determine the renewal award amount if the income is not re-submitted (unless the family qualifies for low income status, in which case the income verification form must be completed annually).
If a family’s household income is at or below 450 percent of the Federal Poverty Level, they will be awarded the maximum scholarship amount of $8,408.
The state has also approved a sliding scale based on a family's gross adjusted income. If a family’s household income is at or above 451 percent of the Federal Poverty Level, the student’s scholarship award will be prorated. This means that ALL families that are not Cleveland Scholarship or EdChoice Traditional Scholarship recipients, will be eligible for state funding that will be applied to a student's tuition.
Here are the Federal Poverty Levels (FPL) with Gross Annual Amounts.
Most importantly, here are the Estimated EdChoice Expansion Award Amounts by FPL Ratio.
Those families whose household gross income are at 201% or higher than the Federal Poverty Guidelines must pay the remaining tuition balance. EdChoice Expansion recipients that have a household gross income of 200 percent or lower qualify for low-income, and Holy Name cannot charge the remaining tuition balance (other fees are required to be paid to Holy Name).
Holy Name utilizes FinalForms to streamline the state scholarship process. The parent/guardian will log into his/her FinalForms account to access the state scholarship information. The documents must be submitted on FinalForms as well.

The state of Ohio determines a family's eligibility and scholarship amount. A student's application will not be reviewed until the ODE has
received both the application and all income verification documents.
EdChoice Expansion Renewal Applicants only need to fill out the income verification form if:
A. A family’s 2023 AGI (located on page 1, line 11 of the 1040 form) is at or below 200% of the federal poverty level. “Low income status”
MUST be determined yearly via the income verification process.
B. A family’s 2023 AGI has decreased from last year’s submission and a new award amount would be calculated by the state.

This includes, but is not limited to, Bedford, East Cleveland Shaw, Euclid, Garfield Heights, Maple Heights, Richmond Heights, Warrensville Heights, Elyria, and Lorain High Schools.
Please note that any current Holy Name student in grades 9-11 who received the EdChoice Traditional Scholarship in 2023-2024, is eligible to renew the EdChoice Traditional Scholarship in 2024-2025. Holy Name High School accepts the Cleveland Scholarship Program (CSP). Provided through the Ohio Department of Education, this Program gives students living within the boundaries of the Cleveland Metropolitan School District the opportunity to attend private schools at a discounted rate. The high schools chosen for attendance must be located in Cleveland, or located in a district adjacent to Cleveland. Holy Name qualifies as it is located in the Parma City School District, which is adjacent to Cleveland. Please note that these scholarships are controlled by the state. For high school students, the maximum scholarship amount awarded through the ODE's Cleveland Scholarship Program is $8,408. Families are then responsible for paying the remainder of the tuition balance.
