Cleveland Scholarship Program (CSP)

Holy Name High School now accepts the Cleveland Scholarship Program (CSP). Provided through the Ohio Department of Education, this Program gives students living within the boundaries of the Cleveland Metropolitan School District the opportunity to attend private schools at a discounted rate. The high schools chosen for attendance must be located in Cleveland, or located in a district adjacent to Cleveland. Holy Name qualifies as it is located in the Parma City School District, which is adjacent to Cleveland. For high school students, the maximum scholarship amount awarded through the Cleveland Scholarship Program is $5,700. Families are then responsible for paying the remainder of the tuition balance.

As of February 2018, students and families can begin applying for the Cleveland Scholarship Program. The application deadline for all materials to be sent into Holy Name High School will be in April 2018

If you currently are a Cleveland Scholarship Program recipient, you must fill out the 2017-2018 Renewal Form. After filling out this form, attach a copy of a current utility bill and a copy of your student’s birth certificate, and return it to Holy Name High School, Attn. Cleveland Scholarship Program, 6000 Queens Hwy., Parma Heights, OH 44130.

If you live in the city of Cleveland, but are not currently utilizing the Cleveland Scholarship Program, we encourage you to apply. You must fill out the 2017-2018 Request Form. After it is completed, send the form, along with a copy of a current utility bill and a copy of your student’s birth certificate, to Holy Name High School, Attn. Cleveland Scholarship Program, 6000 Queens Hwy., Parma Heights, OH, 44130.

Additionally, if you believe you and your family will qualify for the low income guidelines given by the Cleveland Scholarship Program, you must also fill out the 2017-2018 Income Verification Form. This form has not yet been made available by the state of Ohio. As soon as it is, a link to the form will be included on this website. After completing this document, mail it directly to the Ohio Department of Education, Scholarship Program Office, 25 S. Front Street, Mail Stop 309, Columbus, OH 43215. Please do NOT return this form to Holy Name.

Holy Name Class of 2021 Scholarships

*Incoming freshmen can only apply for the below scholarships after they have been accepted as a member of the Class of 2021. Acceptance letters will be mailed on Wednesday, February 15, 2017.*

*Unless otherwise noted, all scholarship applications will be due back to the Holy Name Advancement Office by Friday, March 24, 2017.*

Academic Scholarship Award
This scholarship is available to all incoming freshmen who earn an A- or higher in seventh and eighth grade classes. This $1,000 scholarship is renewable annually as long as a student maintains a strong GPA in courses that are suitable for his/her academic level. There is no application as this is offered based on junior high grades.

Alumni Scholarship
This grant is available to all incoming freshmen whose mother and/or father graduated from Holy Name. Each grant is worth $250 and is renewable for all four years. There is no application as this is offered to all students whose parents are alumni.

Merit Scholarship
This scholarship is available to all incoming freshmen who earn As or Bs in seventh and eighth grade classes. This $500 scholarship is renewable annually as long as a student maintains a strong GPA in courses that are suitable for his/her academic level. There is no application as this is offered based on junior high grades.

Music Scholarship
This scholarship is available to all incoming freshmen who audition. Music scholarship audition information can be found in the Holy Name Admissions Viewbook. You can also learn more by contacting Music Director Evan Campus at ecampus@holynamehs.com.

Placement Test Scholarship
This scholarship is available to those incoming freshman students who placed in the top percentiles on the Holy Name Entrance Exam. Award amounts will vary. There is no application as this is offered based on entrance exam scores.

Ciolek Educational Fund
This scholarship provides students with funds to obtain an education at a Catholic high school within the state of Ohio. Applicants must be currently enrolled as freshmen, sophomores or juniors at Holy Name, or must be eighth graders who will be incoming freshmen at Holy Name during the following school year. A minimum of one (1) $1,500 renewable scholarship is awarded per class. All scholarships will be awarded at the class awards assembly held in May, and will be used for the following academic year. Incoming freshmen recipients will be notified via mail.

Class of ’57 Scholarship
This scholarship was established in 2007 by the Holy Name class of 1957 during their 50th reunion year. Applicants must be current eighth graders who will be incoming freshmen at Holy Name during the following school year. One (1) $1,000 non-renewable scholarship will be awarded, and will be used for the following academic year.

Kenneally Family Scholarship
This scholarship was established in 2010 by Terrence ’67 and Susan Cejka ’67 Kenneally, as well as their son, Sean Kenneally ’87. Applicants must be currently enrolled as freshmen, sophomores or juniors at Holy Name, or must be eighth graders who will be incoming freshmen at Holy Name during the following school year. The amount of scholarships awarded, as well as the award amount, will vary from year to year. Scholarship money awarded will be used for the following academic year.

Bernard & Anna Kilcoyne Scholarship
This scholarship was established in 1989 in honor of Bernard and Anna Kilcoyne. Grants will be awarded in the following priority: First, to the direct descendants of Bernard and Anna Kilcoyne, and second, to four non-family members, one each in grades nine, ten, eleven and twelve at Holy Name High School. The number of $500 scholarships awarded varies from year to year. All scholarships are renewable.

Richard Koenig Memorial Scholarship
This scholarship was established in 2016 in memory of Richard Koenig, a devoted family man and dear friend, and father of three HN graduates. Applicants must be current eighth graders who will be incoming freshmen at Holy Name during the following school year. One (1) $750 non-renewable scholarship will be awarded, and will be used for the following academic year.

Thomas P. Talty ’30 Scholarship
This scholarship was established in memory of Thomas P. Talty, a 1930 graduate of Holy Name. Applicants must be currently enrolled as freshmen, sophomores or juniors, or be eighth graders who will be incoming freshmen at Holy Name during the following school year. The amount of scholarships awarded per class, as well as the award amount, will vary from year to year. All scholarships will be awarded at the class awards assembly held in May, and will be used for the following academic year. Incoming freshmen recipients will be notified via mail.

Holy Name Class of 2018, 2019 and 2020 Scholarships

*Unless otherwise noted, all scholarship applications will be due back to the Holy Name Advancement Office by Friday, March 24, 2017.*

*Students who receive the Ciolek, Kilcoyne or Talty Scholarships during their freshman year, as well as Placement Test, Merit, Academic, Alumni and Music Scholarships, will have those automatically renewed each year, as long as they continue to meet the criteria set for each scholarship.*

Thomas Danzey ’96 Memorial Scholarship
This scholarship was established in 1999 in memory of Thomas Danzey, a 1996 graduate of Holy Name. Applicants must have completed their freshmen and sophomore years at Holy Name and be currently enrolled as juniors. Two (2) $500 non-renewable scholarships – one to a female student, one to a male student – will be awarded at the class awards assembly held in May, and will be used for the following academic year.

Frank DiMarino Memorial Scholarship
This scholarship was established in 2012 in memory of Frank DiMarino, a loyal Holy Name supporter and father of four HN graduates. Applicants must be currently enrolled as freshmen, sophomores or juniors at Holy Name. Two (2) $1,000 non-renewable scholarships will be awarded at the class awards assembly held in May, and will be used for the following academic year.

Shannon Fedor ’01 Memorial Scholarship
This scholarship was established in 1999 in memory of Shannon Fedor, a member of the Holy Name class of 2001. Applicants must be currently enrolled as sophomores at Holy Name. Two (2) $1,000 non-renewable scholarships – one to a female student, one to a male student – will be awarded at the class awards assembly held in May, and will be used for the following academic year.

Joseph Franck ’73 Memorial Scholarship
This scholarship was established in 2012 in memory of Joseph Franck, a 1973 graduate of Holy Name. Applicants must be currently enrolled as sophomores at Holy Name. One (1) $1,000 non-renewable scholarship will be awarded at the class awards assembly held in May, and will be used for the following academic year.

Kenneally Family Scholarship

Eugene T. Krakowiak Scholarship
This scholarship was established in honor of Eugene T. Krakowiak, who served as the school’s principal from 1968 to 1996. Applicants must be currently enrolled as juniors at Holy Name. Three (3) $500 grants will be awarded to male students and three (3) $500 grants will be awarded to female students. All scholarships will be awarded at the class awards assembly in May, and will be used for the following academic year.

Emil B. Maras Scholarship
This scholarship was established in memory of Emil B. Maras who taught at Holy Name High School from 1963 until his retirement in 1983. Applicants must be currently enrolled as freshmen, sophomores or juniors at Holy Name. Three (3) $2,000 non-renewable scholarships will be awarded at the class awards assembly in May, and will be used for the following academic year.

Sara Jean McCarthy ’00 Memorial Scholarship
This scholarship was established in 2002 in memory of Sara Jean McCarthy, a 2000 graduate of Holy Name. Applicants must have completed their freshmen and sophomore years at Holy Name and be currently enrolled as juniors. Two (2) $750 non-renewable scholarships will be awarded at the class awards assembly held in May, and will be used for the following academic year.

Karen Powers (Drama) Scholarship
This scholarship was established in honor of Karen Powers, who was a drama teacher at Holy Name. Applicants must be currently enrolled as freshmen, sophomores or juniors at Holy Name, and be active members of the school’s Drama Club. One (1) $500 non-renewable scholarship will be awarded at the class awards assembly held in May, and will be used for the following academic year.

James Priebe ’65 Memorial Scholarship
This scholarship was established in 1996 in memory of James Priebe, a 1965 graduate of Holy Name. Applicants must have completed their freshmen and sophomore years at Holy Name and be currently enrolled as juniors. One (1) $500 non-renewable scholarship will be awarded at the class awards assembly in May, and will be used for the following academic year.

Edward Seitz ’81 Memorial Scholarship
This scholarship was established in 2006 in memory of Edward Seitz, a 1981 graduate of Holy Name. Applicants must be currently enrolled as freshmen, sophomores or juniors at Holy Name. One (1) $1,000 non-renewable scholarship will be awarded at the class awards assembly held in May, and will be used for the following academic year.

Sisters of Charity Scholarship
This scholarship was established in 2002 in memory of Sister Marie Magdalen Dunn, educator and assistant principal of Holy Name High School from 1970 to 1996. Applicants must have completed their freshmen year at Holy Name and be currently enrolled as sophomores. One (1) $750 non-renewable scholarship will be awarded at the class awards assembly held in May. All grants will be used for the following academic year.

Stenson Family Scholarship
This scholarship was established in 2016 to honor the Stenson family. Joseph and Anne Stenson moved to Cleveland in the 1950s to the “old neighborhood” around Holy Name Church. The move was not successful in bettering their lives as Joseph suffered a disabling stroke soon afterwards. They subsisted on the hard work of Anne at many odd jobs and on government aid. Despite this hardship, their three children, Joseph ’61, David ’63 and Anne V ’71 (Vickie), attended Holy Name and put themselves through college and graduate school with scholarships and student loans. The family strongly believes that education was the key to their success.

The annual total award shall be $10,000 (ten thousand dollars) and may be awarded to a recipient in any increment (not to exceed $10,000 in total per annum) at the discretion of the school’s Scholarship Award Committee. Applicants must be currently enrolled as freshmen, sophomores or juniors at Holy Name. Tradition with Vision Scholarship (for HN Elementary graduates only)
This scholarship was established in 1998 to help students at Holy Name Elementary School in Cleveland continue their Catholic education at Holy Name High School. Only those who are currently in the eighth grade at Holy Name Elementary and have been accepted to attend Holy Name High School for the following school year are eligible to apply for this scholarship. One (1) $1,000 scholarship will be awarded to a male student, and one (1) $1,000 scholarship will be awarded to a female student. Scholarship recipients will be notified via mail.

CHALTA Sprungle Memorial Scholarship
This scholarship is offered by the Cleveland High School and Academy Lay Teachers Association (CHALTA). Applicants must be current eighth graders who will be incoming freshmen at Holy Name during the following school year. The deadline for submission is Tuesday, April 11, 2017, and applications should be mailed back to Holy Name High School, Attn. Ms. Katie Hennessey, 6000 Queens Hwy. Parma Heights, OH 44130. Two (2) $500 scholarships will be awarded. Recipients will be notified by letter by Friday, June 9, 2017.

James E. Flynn Memorial Scholarship
Applicants must be currently enrolled as freshmen, sophomores or juniors, or be eighth graders who will be incoming freshmen at Holy Name during the following school year. Five (5) high school scholarships at $1,000 each will be awarded. The scholarships are awarded by a random drawing, with the winner being selected at the regular monthly meeting of the Board of Directors in April of each year. All funds will be paid directly to the school, on behalf of the student. Students must be an active member of the Unity Catholic Federal Credit Union, must be attending a Catholic school within the Diocese of Cleveland and Eparchy of Parma and must fill out a separate scholarship application form. Applications must be received by April 1, 2017, and sent to Unity Catholic Federal Credit Union, Attention: Scholarship, 5839 Ridge Road, Parma, OH 44129. Please do NOT turn this scholarship in to the Holy Name Advancement Office; send directly to the address listed here.

St. Elizabeth Ann Seton Council 11464 Knights of Columbus Ferdinand P. Goll Memorial Scholarship
This scholarship is offered to all students who are enrolled in a Catholic high school located within the Cleveland Diocese. One (1) $500 scholarship will be awarded. Applications for 2017-2018 have not yet been given to Holy Name from the Knights of Columbus. Once that information is available, it will be posted online.
