The below sections will walk you through how to complete the items on the first checklist found in your acceptance box. In the coming months, we will provide you with more information, upcoming events, and additional checklists so that you are set for the first day of high school!   • On Thursday, February 26th at 6:00 pm in the Main Gymnasium, we will host our Freshmen Welcome meeting. RSVP HERE

    During this meeting you will meet members of the Holy Name staff and fellow students! There will be additional information regarding registration for the 2026-2027 school year, and the team shop will be open!

  • FinalForms is Holy Name's online forms and data management service. To begin registering your student at Holy Name, you must create an account as soon as possible. Please register at https://holynameparmaheights-oh.finalforms.com/ and follow the prompts to create your account, create your student's account, and sign your forms.


    • We are asking that the first 3 form sections be completed by the Welcome Meeting on Thursday, February 26th. These sections are: Contact Information, Demographic Information, and Additional New Student Information.

      Feel free to complete as much as possible, but in order to register for classes at your student's guidance counselor appointment, the 3 sections listed above must be completed.

      *Note: If you have a current Holy Name student and are already using FinalForms, please do not create a new account. You will only need to 'add' your new student to an existing account.

      **Note: Your student will receive a student account in August at Freshmen Orientation. Please do not worry about entering a student email at this time.

  • Select an appointment to meet with your guidance counselor to register for classes. Registration information can be found below! Guidance Counselors are assigned and grouped by student last name.

  • Be sure to add any sports your student may be interested in to your Final Forms account. This makes it easy for the coaches to send out information regarding summer workouts. If you have questions, contact Ms. Kim Luthman at kluthman@holynamehs.com. Registration for Winter & Spring sports will open during the school year.

  • More information will be shared for additional recommended school supplies. 

  • Download the 2026-27 Course Catalog to familiarize yourself with the HN curriculum and graduation requirements.

    You may also reference the current 2025–26 Student Handbook, which includes dress code, student policies, academic standards, and more. Please note that the handbook will be updated for the 2026–27 school year this summer.


  • The Team Shop will be open the evening of the Welcome Meeting! Additionally, you can purchase Holy Name apparel online, year-round, at https://sideline.bsnsports.com/schools/ohio/cleveland/holy-name-high-school.
